Our Service
Our mission is to support individuals in improving and maintaining their overall well-being by providing access to nurse-led information, education, support, and advice. This service is offered to members of the Belfrey congregation, our social action transformation partners, and beyond.
Our Parish nursing service does not replace the care provided by statutory services; rather, it complements them.
We work in compliance with these services and support individuals by walking alongside them to help achieve any existing guidelines, aims, or objectives they may already have.
The role of the parish nurse includes the following:
Receiving referrals and triaging to identify needs across all ages as we navigate the transitions of life.
Seeing clients at our Belfrey location as well as home visits.
Assist the Belfrey pastoral team in crisis management by offering advice and supporting appropriate onward referrals.
Signposting and support to access help and help to navigate NHS referral pathways.
Provide health education, advice on healthy lifestyle changes.
Help and support to understand common mental health conditions.
Support with spiritual aspects of health and well-being.
Offer support for people experiencing isolation and loneliness.
